My Aged Care
Home Care Package
Overview
Home care package (HCP) is provided by My Aged Care. It funds support services for older people and people with disabilities, to help them continue to live in their own homes.
HCP are tailored to meet the individual needs of the person, and are designed to help them remain independent, safe and comfortable in their own home. HCP can include services such as:
-
Personal care and support
-
Assistance with activities of daily living
-
Home maintenance
-
Meal preparation and delivery
-
Transport
-
Social support and outings
-
Assistance with medications
-
Assistance with shopping and errands
-
Coordination and management of care
HCP are available at four levels, starting with Level 1 for basic care needs and up to Level 4 for high-level care needs. The level of package is determined by the individual's assessed care needs. The Australian Government contributes a different amount for each level of HCP. Your total HCP budget depends on your level of care, and includes contributions from you and the government.
To be eligible for HCP, a person must be assessed by a government-approved assessment team as having a care need, and be over the age of 65 or have a disability. It's important to note that the availability of HCP is limited, and the waiting lists for these packages can be quite long.
